This is the formal intake form to request a project with IT Services. Submit this request if you are implementing new applications or systems that require multiple ITS Divisional resources.

What Happens once I submit this form?
The Project Management Office will review your request and conduct an assessment. The Office will contact you with any question to complete the assessment to determine if your request meets project requirements to move through the review process for approval.
